Ask us to communicate with you in a specific way or at a different location.
Receive an Accounting of Disclosures
Request a list of certain disclosures of your PHI that we have made outside of treatment, payment, and healthcare operations.

If you believe your privacy rights have been violated, you may file a complaint with Rhema Wellness Institute, PLLC or with the U.S. Department of Health and Human Services. Filing a complaint will not affect the care you receive.

Electronic Records and Communication.

Rhema Wellness Institute, PLLC may maintain health records using secure electronic systems and may utilize secure technology for scheduling, telehealth, client messaging, documentation, and related services.

While we use reasonable administrative, technical, and physical safeguards to protect your information, no method of electronic communication or storage can guarantee absolute security.

Email and text messaging may be used only with your consent and should not be relied upon for emergencies.

Business Associates

We may work with trusted third-party service providers who perform services on our behalf, such as secure electronic health records, payment processing, or technology support. When required by law, these providers are bound by Business Associate Agreements that require them to protect your PHI in accordance with HIPAA.

Minors

When services are provided to minors, parents or legal guardians generally have rights regarding access to a minor's PHI, subject to applicable federal and Texas law and recognized exceptions.

Retention of Records

Clinical records are maintained and retained in accordance with applicable federal and Texas laws, licensing requirements, and professional standards.

Changes to This Notice

We reserve the right to change this Notice at any time. Any revised Notice will apply to all PHI maintained by Rhema Wellness Institute, PLLC and will be posted on our website and made available upon request.
